We are looking to recruit an experienced and highly motivated advanced clinical practitioner, either a nurse or allied health professional (AHP), to join the Frailty Virtual Ward team. The role will primarily involve working closely with the clinical lead, liaising with general practitioners and community teams to assess patients who meet the criteria for the frailty virtual ward. The post holder will contribute to the development of individualized care plans and provide expert management of medical conditions, ensuring patients receive high-quality, personalized care. Additionally, the post holder will apply their expertise to safely manage patients in the comfort of their own homes, helping to improve outcomes and enhance the quality of life for those with frailty.

Main duties of the job
1. Contribute to the development, implementation, and review of personalized care plans for frail patients, ensuring they are tailored to individual needs and preferences.
2. Work closely with the clinical lead to adjust care plans as required, ensuring optimal management of both acute and long-term medical conditions.
3. Promote a holistic approach to care, considering both medical and psychosocial aspects of the patient's health.
4. Provide advanced clinical decision-making, diagnosing, and prescribing within the scope of practice to manage medical conditions and frailty symptoms effectively.
5. Monitor and manage frailty-related conditions, such as falls, mobility issues, cognitive decline, and chronic disease management, ensuring patients' needs are met in both acute and community settings.
6. Liaise regularly with general practitioners, community teams, and other healthcare providers to ensure continuity of care for frail patients.
7. Provide professional guidance and support to community teams and junior healthcare staff in managing frailty within the virtual ward framework.
8. Utilize digital tools and remote monitoring systems to track patients' conditions, providing virtual consultations and interventions when appropriate.
9. Support patients and their families in managing their conditions remotely, ensuring they receive the necessary support to remain safely at home

Qualifications
Essential criteria
10. Registered Nurse with NMC registration or Therapist with relevant skill set and HCPC registration
11. Masters in Advanced Clinical Practice
Desirable criteria
12. Relevant training/qualification in a specific area of neurosciences older people’s nursing/ frailty
13. Mentorship qualification
Experience
Essential criteria
14. Minimum of 5 years relevant post-registration clinical practice experience
15. Experience as Band 7/Ward management in Frailty or working with older people.
16. Experience of managing a clinical team including performance management and appraisals
17. Experience of service development and improvement
Desirable criteria
18. Experienced Band 8A advanced Clinical Practitioner
19. Minimum of 2 years of Clinical Research experience in NHS, University and commercial clinical trials environment
Knowledge
Essential criteria
20. Expert knowledge in CGA, frailty and care of the elderly.
21. Foundation knowledge of service improvement methodologies
22. Evidence of continuing professional development activity to maintain up to date knowledge and competence in all aspects of the role
23. Advanced physical assessment course
Desirable criteria
24. Knowledge of professional and political issues in healthcare
Skills

25. Work as a competent practitioner in acute and community environments and acts as a clinical role model.
26. Possess advanced patient assessment skills and demonstrates specialist knowledge in practice Is resilient when dealing with complex situations

28. As an Advanced Clinical Practitioner demonstrate the ability to develop and use novel approaches to practice which are appropriate to the needs of the client/patient group and consistent with the aims of the Trust.
