Job summary
This is a development role, and as such the post holder will be expected to complete the MSc in Advanced Clinical Practice with Independent Prescribing upon commencement of the role. Note this would not be expected during a secondment/fixed term contract.
During this period, salary will be paid in accordance with Annex 21 of the Terms and Conditions as follows:
1. Year 1 of the training period will be paid at 65% of the top of Band 7 pay scale.
2. Year 2 of the training period will be paid at 70% of the top of Band 7 pay scale.
3. Year 3 of the training period will be paid at 75% of the top of Band 7 pay scale.
Upon successful attainment of the MSc and Independent Prescribing qualifications, the post holder will automatically move to the bottom of Band 7, as they will then meet the full KSF outline of this role, and be expected to undertake all duties within the Job Description as indicated.
Main duties of the job
The post holder will work within the Critical Care Outreach team to ensure quality of care. The main aims of this position will be to enhance the standards of care for critically ill patients; irrespective of where they are being nursed.
The post holder will be expected to maintain a portfolio of advanced practice to demonstrate their work against the four pillars of advanced practice.
The post holder will be required to ensure the highest standard of patient care, which reflects the quality standards and mission of the University Health Board and contribute to the implementation of policies and procedures within their sphere of responsibility.
The post holder will plan implement and evaluate care according to changing needs and varying complexity of the patient.
The post holder will prioritize and deliver care in collaboration with other health care professionals.
The post holder will involve patients and where appropriate relatives and carers in planning and delivering care.
The post holder will facilitate information exchange of patient related conditions and treatment at the end of the shift.

The post holder will work within the multi-disciplinary Outreach Team, to deliver a high standard of patient care to the identified Outreach patients.
The aim of the Outreach Team is to:
* Avert admissions to Level 2/3 care.
* Facilitate discharge from Critical Care.
* Share Critical Care skills.
