We are looking for 2 full time (37.5 hours per week) Band 4 Therapy Assistant Practitioners to join the amazing Acute Medicine Therapy Team at Peterborough City Hospital.

Experience of working as a therapy assistant or in a hospital/community/rehabilitation environment is essential. A willingness to learn and develop is essential along with good communication skills and an ability to work in a team.
The role involves;
• Supporting patients with exercise programmes and mobility
• Assisting therapists with assessments and discharge planning
• Completing functional tasks to support patients in returning to their previous level of function
• Administrative duties
• Visits to patients homes to assess their discharge needs
• Assessment, practice and provision of equipment.

This role requires the ability to travel between sites. A valid driving licence and access to a vehicle may be necessary for this role. However reasonable adjustments will be considered for candidates who can meet the travel requirements through an alternative means.
Interviews for this role will be conducted in person to assess relevant skills effectively.
• To undertake assessments and treatments for patients that have been triaged by a Therapist and deemed appropriate for Therapy Assistant Practitioner intervention with case management advice available via indirect and direct contact with a registered therapist.
• To carry own caseload of non-complex patients assessing, according to competencies, and devising and implementing treatment programmes.
• To work collaboratively within Multidisciplinary and Multi-agency teams with the aim of supporting timely discharge.
• To enable patients, their families and carers in the attainment of their rehabilitation goals by providing treatment programmes developed and monitored by Occupational Therapists and Physiotherapists in supervision.
• To deliver safe, efficient and effective patient focussed care in line with local standards.
• To support managing the Therapy referral line.
• To be actively involved in the in-service training programme, including the mentoring and training of Band 2 and 3 therapy assistants and supporting students as required.
• To participate in the provision of 7-day services as required.
• To adhere to the Trusts values and behaviours.
