Job Summary
An exciting opportunity has arisen to recruit an enthusiastic and motivated NMC Registered Band 7 Nurse to join our innovative Care Home Intensive Support Team (CHIST).
CHIST provides Nursing and Occupational Therapy staff for targeted, time-limited support to specified care homes as prioritized through our local system escalation processes. This role is aimed at enhancing the overall care delivered by the CHIST Service.
You will have specialist clinical skills in Nursing to enhance the health and wellbeing of care home residents, providing hands-on clinical support, advice, and guidance based on residents' needs.
You must have a desire to work collaboratively with patients and key partners to facilitate the best possible patient experience and outcomes. The role is community-based with remote working, and you will be required to meet travel demands of the role by working across the county in care homes registered with Staffordshire County Council.
Main Duties of the Job
As a Clinical Lead Nurse joining our service, you will be expected to provide support and advice with personalized care planning, nutritional advice/signposting, and navigating links with other Health and Social Care services.
As part of this role, you will also be expected to:
* Have excellent interpersonal and communication skills and the ability to quickly develop effective relationships with care home staff.
* Undertake tasks concerning the day-to-day organization and operation of the team, working with the AHP Clinical Lead as required.
* Be skilled in risk management and demonstrate clinical leadership skills with the ability to prioritize the support needed in a care home and escalate any issues appropriately.
* Promote effective team working in care homes and have a willingness to share your experiences and knowledge with others, in accordance with evidence-based practice.

Job Responsibilities
This is a dynamic post that requires the post holder to work closely with the multi-disciplinary teams/agencies in addressing a range of complex care issues, preventing unnecessary hospital admissions, and improving the health and wellbeing of the population served. The nature of the service will be determined by local service specifications.
Person Specification
Qualifications & Training
Essential:
* Degree in nursing or equivalent.
* Registration with NMC.
* Post Graduate studies in relevant nursing field such as end-of-life care, long-term conditions/palliative care, or related/relevant subject.
* Mentorship qualification (998-997).
Desirable:
* Modules at Masters level.
* Evidence of further clinical study.
Experience
Essential:
* Relevant experience to the role including knowledge and management of long-term conditions.
* Ability to manage time and workload of both the team and self effectively.
* Working in a multidisciplinary team.
* Knowledge and experience working within both nursing and residential care settings.
Desirable:
* Ability to negotiate and manage conflict in clinical situations.
* Ability to lead and support staff.
* Ability to support in service improvement.
Skills, Knowledge & Abilities
Essential:
* Evidence of specialist clinical knowledge relevant to the post.
* Understanding of Information Governance, Data Protection, NMC standards, and Safeguarding Vulnerable Adults.
* Highly specialized clinical skills in a wide range of nursing knowledge to enhance the health & wellbeing of care home residents.
* Awareness of current guidance in relation to Covid-19, outbreaks, and infection prevention.
* Advanced skills in risk management and the ability to prioritize the support needed in a care home and escalate any issues appropriately.
* Awareness of relevant national policy and guidance.
* Basic IT skills.
* Commitment to evidence practice, excellent clinical leadership skills, and willingness to take on additional responsibilities essential to the ongoing development of the team.
Desirable:
* Understanding of the wider NHS agenda and its application to the local context.
* Understanding/knowledge of the governance and regulatory frameworks pertinent to care homes.
