We are looking for a dynamic and highly motivated Specialist Respiratory Practitioner (Band 6) to join our innovative Respiratory Virtual Ward Specialty team. This is a unique opportunity for either a registered nurse or physiotherapist with a passion for respiratory care to deliver high-quality, patient-centred management in a pioneering community environment.
The Virtual Ward model allows patients with chronic respiratory conditions, such as COPD or asthma, to receive acute care from the comfort of their own homes, avoiding hospital admission and enabling them to manage their condition with the support of an established multi-disciplinary team offering both in-person and remote visits. You will also play a critical role in supporting patients with early discharge, providing clinical expertise, and ensuring continuity of care in the community.
Main duties of the job
We are looking for a compassionate and adaptable professional who thrives in a fast-paced, technology-enhanced care environment. You will need excellent clinical skills, a strong understanding of respiratory conditions, and the ability to work autonomously while being part of a multidisciplinary team.
Key Responsibilities:
* Provide specialist assessment and care for patients with respiratory conditions, including COPD, asthma, bronchiectasis, and interstitial lung disease, within the Virtual Ward setting.
* Work with the Virtual Ward hub team who monitor patients remotely using telehealth systems and clinical assessments, ensuring early recognition of deterioration and timely interventions.
* Utilise point-of-care testing to support clinical decision making.
* Develop and implement individualised care plans in collaboration with the wider multidisciplinary team, including consultants, GPs, and other community services.
* Deliver specialist advice, education, and support to patients and their families, empowering them to manage their conditions at home.
* Work closely with the Virtual Ward team to review and monitor patient progress, adjusting care plans as necessary.
* Participate in the training and development of colleagues, promoting best practices in respiratory care and virtual ward delivery.
* Ensure compliance with clinical governance, patient safety protocols, and quality assurance processes.
