Job Description Responsibilities include: Conduct client Service Reviews and Quality Assurance calls with existing clients and follow up on outcomes. Create, update and audit client care plans and needs assessments using our digital care planning system. Maintain accurate client and Care Professional records on Home Instead software. Conduct client and Care Professional introductions. Build and maintain positive relationships with potential and existing clients, their families and other professionals involved in their care. Support and mentor Care Professionals with support visits and supervisions to ensure exceptional care delivery. Participate in on-call rotation. What's in it for you?
