The Role
The service provides a stepping stone to independence, by supporting people to regain lost skills, learn new skills, and adapt to the challenges that living independently can present.
You will work within a CQC “Good” rated Home Support Reablement Council Team, and will have an opportunity to work with other professionals such as our colleagues in the NHS, Social Workers, Occupational Therapists, etc., to provide all aspects of care, which includes but is not exhaustive of:
1. Moving and Handling
2. Personal Care
3. Administering medication
Ensuring service users' dignity and wishes are at the centre of the care provided.
Home Support Reablement is a short-term service, meaning there is a quick turnover of service users.
The role of a support worker is to support people in their own home who are at risk of needing social care or have an increased intensity of care. Support workers will support service users to regain functioning, maintain life skills, rebuild their confidence, and promote wellbeing.
28-hour contracts are available on a rolling rota with flexible approaches to shift patterns, including split shifts. (See attached rota examples)
