CFT Care Ltd is a well-established health and social care provider dedicated to offering high-quality care to individuals with diverse and complex needs. We are currently seeking a compassionate and skilled Complex Care Support Worker to join our team. In this role, you will be supporting individuals with a range of complex health needs, including learning disabilities, mental health conditions, physical disabilities, and personal care requirements.
Role Overview
Key Responsibilities:
* Supporting individuals with complex needs: Provide care and support to individuals with learning disabilities, mental health conditions, physical disabilities, and those requiring personal care.
* Managing complex behaviours: Work with individuals who may present with challenging or complex behaviours, ensuring their safety and well-being while promoting positive outcomes.
* Following person-centred care plans and risk assessments: Adhere to detailed care plans and risk assessments to ensure the best possible care and safety for each individual.
* Supporting day-to-day activities: Assist individuals with tasks such as shopping, meal preparation, cleaning, and maintaining their homes to ensure a clean and accessible environment.
* Administering medication: Ensure medication is administered in accordance with care plans, following proper procedures for medication management.
* Promoting independence and individuality: Encourage individuals to make decisions about their lives and promote their independence and autonomy in line with their care plan.
* Encouraging healthy relationships: Support individuals in developing and maintaining healthy relationships with family, friends, and the community.
* Supporting access to the community: Assist individuals in engaging with local activities, appointments, and social events to enhance their quality of life.
* Supporting outside of the home setting: You may be required to accompany individuals outside the home, including trips to the hospital, holidays, or appointments at police stations when necessary.
* Reporting to the Service Manager: Regularly report to the Service Manager to ensure care delivery is in line with company policies and regulatory standards.
Desired Skills and Qualifications:
* Experience in supporting individuals with complex care needs, particularly those under DoLS (Deprivation of Liberty Safeguards).
* Strong understanding of person-centred care and risk management.
* Ability to manage and support individuals with challenging or complex behaviours.
* Compassionate, patient, and empathetic approach to care.
* Excellent communication skills, both verbal and written, with the ability to build rapport with individuals and their families.
* Ability to work independently and as part of a team.
* NVQ Level 2 or 3 in Health and Social Care (or equivalent) is desirable.
* Knowledge of relevant health and safety regulations and safeguarding procedures.
* A full UK driving licence (preferred but not essential).
* Flexibility to work varying shifts, including weekends and holidays.
