Dorset HealthCare are looking for a friendly and professional administrator to join our Health and Social Care Co-Ordinator’s based within the Shaftesbury Integrated Community Rehab Team.
The Shaftesbury Integrated Community Rehab Team is a multi-disciplinary team that provides rehab assessment and therapy to both the local community around Shaftesbury and also the Wards within the hospital itself. The team´s goal is to help reduce hospital admission and to facilitate appropriate discharge for any residents within the area. We are a busy and friendly team who work together to promote recovery with people with complex health conditions.
The Health and Social Care co-ordinators role is a part-time post of 30 hours per week over 4 days. We can be flexible with how those four days are met; however, one of the days covered will need to be the Friday.
You will be required to have a flexible and adaptable approach with excellent multi-tasking abilities, as well as a good understanding of Microsoft Office, especially in Excel.

The Health and Social Care Coordinator plays a vital role in supporting individuals who are at risk of repeated unplanned hospital admissions or long-term care. This includes individuals who are socially isolated or frail but whose needs do not require the involvement of a registered health or social care professional in a key worker or case manager capacity.
In this role, you will work closely with the frailty team, contributing to population health management and supporting the Hospital at Home team in efforts to avoid hospital admissions. You will act as the first point of contact for service users and professionals making enquiries about the service.
Key Responsibilities Include:
* Coordinating proactive multidisciplinary team (MDT) care.
* Supporting the frailty and Integrated Community Rehabilitation Team (ICRT).
* Chairing meetings, preparing agendas, and distributing minutes to relevant attendees.
This is a dynamic and collaborative position, requiring excellent organisational and communication skills to support the delivery of seamless, patient-focused care.
