An exciting development opportunity has arisen for a Band 5 Community Practitioner to work within the specialty field of a Community Treatment Team based at Dryden Road Clinic covering the west of Gateshead.
Gateshead West Community Treatment Team is a multidisciplinary service which provides assessment and treatment for people experiencing moderate to severe mental health problems. Your role will incorporate working closely with partner agencies/services in delivering effective care and treatment.
In collaboration with the Service User and their families, the post holder will facilitate formulation which then informs a Recovery and Wellbeing focused Care Plan, ensuring regular reviews to monitor progress and adapt the Care Plan as necessary.
Main Duties of the Job
They will work as a member of the Multi-disciplinary Team ensuring that high quality individualized care is delivered which ensures safety and a commitment to promoting resilience, recovery and wellbeing and maximizing independence.
The successful candidate will be responsible for the assessment, management, and coordination of the care of service users who have a variety of complex needs, diverse presentations, and a range of mental health conditions.
Key Responsibilities
1. Assessment of clients presenting with significant mental health needs.
2. Developing collaborative care plans with clients and their carers.
3. Delivering evidence-based interventions to support recovery.
4. Identifying safeguarding concerns and responding accordingly.
5. Assessment and management of risks.
6. Working within an MDT.
7. Completion of electronic records as per policy and professional body.
8. Working with other agencies to provide appropriate care and treatment.
9. Engaging in supervision processes.
Person Specification
Education and Qualification
* Experience of working with service users and carers in successful delivery of treatment plans.
* Multi-disciplinary team work.
* Proven ability to develop therapeutic partnerships with service users and carers.
* Proven ability to communicate interventions to service users.
* Phlebotomy trained.
Knowledge and Experience
* Knowledge of relevant contemporary professional theories of mental health care and evidence-based interventions.
* Experience of working with adults with mental health difficulties.
* Experience of working within a community setting.
* Demonstrates knowledge and experience of psychosis.
* Experience of working within an MDT.
Skills and Competencies
* Ability to use IT systems.
* Comprehensive continuous assessment of skills.
* Risk Management and Safety Planning skills.
* Ability to establish rapport and demonstrate engagement skills with service users.
* Knowledge and understanding of therapeutic interventions to promote recovery.
