Clinical Responsibilities: To assess, develop, implement and evaluate programmes of care. To autonomously initiate emergency interventions as patient/client condition dictates. To provide prescribed nursing care without supervision, within the level of the post-holders experience, referring to a more senior nurse when necessary. To participate as team member, including supporting relatives and friends of patients/clients together with providing support for other patients/clients. To maintain accurate records, both electronic and paper, of the care given in accordance with agreed standards. To act in accordance with the NMC Code of Professional Conduct and maintain NMC registration. Independently liaise and communicate effectively with members of the nursing staff, multi-disciplinary team and other associated agencies. Demonstrate evidence-based practice. Act as Care Co-ordinator for patients/clients on CPA (Care Programme Approach). Participate in team reflective practice sessions. To participate in and contribute to team meetings and clinical reviews. Training and Education: Participate in contributing to the continuing development of the departments philosophy, training/education. To undertake annual statutory and mandatory training as identified by the Department. To undertake the role of health promoter and educator for patients/clients and their families/significant others. To maintain and develop own skills to promote evidence-based practice, within the role of providing community-based care. To participate in clinical supervision. To assist in the learning, development and assessment of unqualified staff, junior staff and adaptation nurses. To undertake a formal mentorship role if an appropriate qualification is held. To undertake training to continue professional development. Leadership/Managerial: Deputise for the team manager in their absence providing leadership and support to the team. Attend any service led meetings as required. Act as a role model for all staff, maintaining professional boundaries and relationships. In liaison with team manager, gain experience in providing leadership to junior staff. Provide supervision and case management support to junior staff. Provide support with support workers undertaking VQ award. Communication: To co-ordinate complex care arrangements through communication with multidisciplinary team members and associated agencies. To give due consideration to language and cultural differences of both patients/clients/relatives/significant others and colleagues. To communicate effectively with patients/clients and significant others. This could include patients/clients with reduced consciousness levels (including intoxication and trauma), violent and aggressive behaviour and support the bereaved. Audit/Information Technology: To participate in clinical audit as required in department. With training, access organisational electronic information systems relevant to the role. Participate in agreed data collection and reporting processes.
