Nurse Consultant - Frailty Service (Aintree Site)

An exciting opportunity has arisen for a nurse/AHP consultant practitioner with a passion for frailty.
We are looking for a dynamic, strategic thinker with experience of working in Frailty to provide leadership and clinical expertise in the development of our Same Day Emergency Care and expansion of the existing ambulatory pathways.
You will become part of our expanding, exciting, and outward-looking team and will have the opportunity to work together with the multi-disciplinary team to make a difference in the care of our Frailty patients.
The successful candidate will take the lead in driving the ambitions for the Frailty service, as well as developing links with partners to allow direct admissions from the community to the frailty service and developing our established ambulatory service.
The suitable candidate would have experience of working within a Frailty or Geriatric service and have either or both community or acute hospital experience.
The suitable post holder will have experience of leading change, audit, research, and education.
This is a full-time, 8b post and we are looking for someone to provide leadership in the development of the 7-day service of the Frailty team.
Main duties of the job
The Nurse/AHP Consultant will work as an expert autonomous practitioner providing nursing assessment advice and interventions for patients by:
1. Leading a co-ordinated and efficient service to patients.
2. Taking an active role in the education, training, and development of Health Care Professionals to promote the objectives of the service.
3. Promoting early intervention with the most appropriate form of treatment/support, which will lead to more effective management of the disease process and an improvement in the patient's quality of life.

Qualifications
* Masters Degree
* Active Professional Portfolio/relevant registration
Experience
* Demonstrable clinical experience in the management of patients within the area of specialism
* Recent experience of service development
Knowledge
* Clear understanding of nursing and policy issues
Skills
* Development of strategy
* Presentation at local and national level
* Ability to provide comprehensive advice and support to patients and the health care team
* Audit and Research skills
