Job summary The Gateshead Learning Disability Team are happy to be offering an exciting opportunity for Band 5 Community Nurse. We are a multidisciplinary community team working across the whole of Gateshead with individuals with complex needs. The successful post holder will be expected to demonstrate a strong commitment to high quality patient care, good organisational skills and be able to demonstrate prioritisation of workload. Key components of the role will be to ensure the service offer a safe, caring, responsive and well-led service. The role will include liaising with agencies such as social care, service providers, inpatient services, safeguarding, alongside a range of other agencies. Main duties of the job To collaboratively engage in the assessment and management of a caseload of patients who have substantial and complex needs. Provide clinical advice to patients, families, carers, staff, statutory agencies, third sector partner agencies and the public. You will be expected to provide high quality assessment and care, alongside offering the wider team support, advice and supervision as required. Work collaboratively with patients to develop care plans, recovery and wellbeing plans. Ensuring patients have access to regular reviews to monitor progress and adapt care plans as necessary. To consider carer needs and ensure carer and family are involved in care planning where appropriate. To deliver evidenced based clinical interventions. The post holder will work as a member of the Multi-disciplinary Team ensuring that high quality individualised care is delivered which ensures safety and a commitment to promoting recovery and wellbeing and maximising independence. Coordinate treatment planning, interventions, clinical reviews and discharge planning. Provide clinical supervision to staff as delegated by Pathway Manager and Clinical Leads.
