St Elizabeth Hospice is an independent charity and hospice, which improves life for people in the East Suffolk, Great Yarmouth and Waveney areas living with a progressive or life-limiting illness. Established in 1989, the hospice has built a reputation for delivering high-level care to patients and the community it serves. Through medicine and therapy, they ease pain; give life purpose and make life livable. Last year the charity supported over 4,000 patients and their families.

Main duties of the job
Our Care Coordinators work alongside our clinical teams to coordinate patient services in our palliative care coordination hub. The team covers all aspects of work in the hub including being the first point of contact for callers to our 24/7 advice line OneCall. Our team cover shifts on a four week fixed rota, 7 days a week including Bank Holidays, with shifts covering from 8am to 8pm. Full training will be provided.
Salary: WTE £24,729.00 to £25,365.00 plus enhancements for unsocial hours

JOB SUMMARY
St Elizabeth Hospice provides planned, reactive and coordinated care to patients requiring palliative and end of life care. The care coordinators cover 7 days per week, supporting our palliative care coordination hub, including community, medical, therapy, and emotional wellbeing teams.
SAFEGUARDING RESPONSIBILITIES

* Maintain awareness of hospice safeguarding policies
KEY RELATIONSHIPS INCLUDE:
* Patients, families, and informal carers
* Community, medical, therapy, and emotional wellbeing teams
* Head of Nursing, Suffolk Continuing Healthcare Team, Primary Care teams, hospice multidisciplinary team, community volunteers, Marie Curie, Social Services, Care Agencies, local bereavement services, funeral directors, hospital discharge coordinators, data personnel
RESPONSIBILITIES
OneCall
Provide support to the advice line, handle initial calls, and liaise with services to meet patient needs.
Triage
Process new referrals, liaise with healthcare professionals and patients, ensure records are complete, and coordinate appointments.
Care/Virtual Ward
Signpost, provide information, coordinate visits, and organise care packages for end-of-life patients.
Community Volunteers
Understand volunteer services and support the lead coordinator in their absence.
General
Support understanding of hospice units, maintain accurate records, inform families and professionals of updates, participate in meetings, and handle sensitive information appropriately.
Professional Development
Develop a personal development plan, maintain professional skills.
Personnel Responsibilities
Recognize staff stress, participate in induction and placements, support volunteers.
Educational Responsibilities
Provide feedback on training, support team development.
Organisational Responsibilities
Work within policies, maintain confidentiality, promote hospice values, and ensure resource efficiency.
