We are seeking to recruit an energetic, flexible, and enthusiastic practitioner in the Crisis Resolution and Home Treatment Team based at Hall Court in Telford. The successful candidate will join the team in providing 24-hour rapid response and assessment of mental health crises in the community, with the possibility of offering acute psychiatric care at service users’ homes or other community facilities until the crisis is resolved. The team covers the geographical area of Telford & Wrekin.
Working in an identified adult community mental health pathway, with the flexibility of working across pathways, dependent on skill and service need.
Responsibilities include:
1. Delivering a high-quality clinical service to patients and their families.
2. Care coordination of an identified caseload with access to regular and robust supervision.
3. Delivering occupational therapy interventions in accordance with an agreed care plan to an identified group of people experiencing mental health-related difficulties.
Communication skills are an integral part of the role. Written and verbal skills will be vital in ensuring that information is documented, shared, and recorded in an accurate and prompt manner.
It is essential that you possess the skills to work as part of a team and have the confidence to work autonomously. You should possess the skills and enthusiasm to take on the challenge of a changing, modern-day community mental health service.
Having proficient IT skills and being a car driver are essential.
