Job Title: Social Worker (Hospital Discharge Team) Location: Middlesbrough Metropolitan of Lewisham Rate: £284 per day An exciting job role has recently become available at the Middlesbrough Metropolitan of Lewisham. They are looking for a new member for their Hospital Discharge Team. X4 workers required for a 12 month period 2 years post-qualifying experience required. Request to cover case work/reviews. This will involve the ability to undertake face-to-face visits 5 days a week with 2 days a week working from an office setting. Learning Disability Team – adults in transition and over 18 with a learning disability.
