Job summary Working as part of a team with individual responsibility for providing efficient and effective clerical support for My Medical Record including but not limited to; Ensuring accurate data entry onto prostate cancer follow up systems. Access system and respond to any patient messages, escalating to nursing staff as appropriate. Generate and send out results letters to patients following confidentially and data protection guidelines. Main duties of the job Accurately input maintain and update patient information into the My Medical Record system. Transfer patient data onto the My Medical Record system from current data bases. Update patient details e.g., change of address, Surname, GP etc on all patient information systems used by Velindre NHS Trust Comply with procedures relating to incident reporting and ensure information is accurately reported. Generate patient letters from the My Medical Record system and send to patients in a timely way. Support the urology CNS in setting up and running virtual and face to face education sessions. Contribute to the day to day running of the urology Clinical Nurse Specialist Team, taking and responding to phone calls, emails and patient enquires. Support the Clinical Nurse Specialist team in keeping educational resource up to date on the My Medical Record System Support the Cancer navigators in keeping all patient information resources up to date and organising new patient packs to be send out to patients. Additional administration tasks as directed by the Team Leader or Health Records Manager.
