We have a permanent post available in our Planned Community Therapy Service. We are a dynamic service providing patient–centred assessment, treatment and rehabilitation to adults in Coventry. Referrals are received from GPs or other community clinicians. The clinical caseload is varied and interesting and can include a mix of conditions that includes; neurological, musculoskeletal, falls, orthopaedics, and management of long term conditions. Every patient is unique, and no two days are the same We are here to promote patients achieving goals that are meaningful to them in their own environment. This can vary from promoting and improving someone’s independence to improving quality of life through education, maintenance, or preventative work. In addition to our home-based rehabilitation, we run a falls prevention service ‘Strong and Steady’. This is a 6 week programme offering education, advice and a tailored exercise programme which is exciting to be a part of. The team consists of Physiotherapists, Occupational Therapists, Dietician, Assistant Practitioners, Technical Instructors and support from psychology. We are a supportive, dynamic team offering robust supervision as well as informal discussions to support each other with caseload management. There is a regular in-service training programme as well as peer group support.
