The post holder is responsible for the delivery of general practice nursing to the practice population. This includes the delivery of evidence- based practice for patients with long term conditions and the provision of preventative healthcare. As an autonomous practitioner the post holder will demonstrate critical thinking and skills in clinical decision-making in the management of patients. Working in accordance with the NMC Code of Professional Conduct, they will work collaboratively within the general practice team to meet the needs of patients, support the delivery of policies and procedures, and provide nurse leadership and support as required. Clinical Responsibilities Deliver treatment programmes for patients with a known long-term condition, or those at risk of developing a long term condition. Work with the full Practice team to diagnose, monitor, manage and treat long-term conditions, including non-drug-based treatment methods using a management plan, and in line with national and local policies and practice needs Review patient needs and treatment holistically in accordance with evidence-based practice and national and practice protocols Individualise care and involve patients in decisions to support the delivery of person-centred care Prioritise health problems and intervene appropriately to assist in complex, urgent or emergency situations, including initiation of effective emergency care Support the care and review of patients in relation to asthma, COPD, diabetes, hypertension, cardiovascular disease, contraception, cervical cytology, adult and childhood immunisations, weight management, mental health and learning disabilities. Work together to promote patients to live healthily and apply principles of self care Provide care provision for housebound patients A strong commitment to working within and promoting best practices in safeguarding both children and adults Communication Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating Demonstrate sensitive communication styles to ensure patients are fully informed and consent to treatment Maintain effective communication with practice colleagues and the wider primary and secondary care teams Maintain confidentiality and GDPR requirements Delivering a high quality service Work within infection control guidance and policy, ensuring safe care provision Work within own competence and professional code of conduct as regulated by the NMC, ensuring maintenance of professional registration Provide care that is based on up to date evidence, following local and national policy and guidance, ensuring regular updates to ensure the best care outcomes Be part of practice clinical governance measures including audit, training, education and quality improvement Maintain accurate record keeping for care provided Complete all mandatory training packages as dictated by the practice and CQC requirements Personal and People Development Actively promote the workplace as a learning environment for all, encouraging everyone to learn from each other and from good practice Contribute the development and update of local policy, procedure, protocols and standards to ensure the best care for our patients Support and actively encourage the development of colleagues Participate in an annual and interim personal development review/appraisal Delegate appropriately to non-registered team members Work well with team members to develop high quality nursing care Disseminate learning and information gained to other team members
