Launceston Close Surgery in Winsford is looking for an experienced ANP for 1 day per week to join our friendly practice team. Our ideal candidate will have experience in general practice. The candidate will ideally be educated to masters level in advanced nursing and hold an independent prescribing qualification.

You will work as part of the primary healthcare team providing General Medical Services to the patients of the practice. The post holder, as the first point of contact will be a caring, compassionate, and a committed experienced nurse who acting within their professional boundaries will provide care that includes but is not limited to history taking, clinical assessment, diagnosis, treatment, prescribing and future care plans. You will instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret complex findings/reports within the scope of an ANP and discuss results with patients where appropriate. You will provide holistic care to the registered patient population and their families.

Overview of your organisation
The practice has approximately 5,200 patients and we are high achievers for QOF and other patient related and contract related targets. The practice is a very family orientated practice of many years standing and has a variety of healthcare professionals and admin staff including GPs, a pharmacist, practice nurses, a healthcare assistant, a GP Assistant, plus reception and admin staff.
We are one of 5 practices who form part of Winsford Primary Care Network and work closely with the other 4 practices to provide a range of services across a total PCN population of approximately 36,000 patients in Winsford. Our PCN includes Pharmacists and Pharmacy Technicians, Physiotherapists, Care Co-ordinators, Social Prescribing Link Workers and GP Assistants.

Job responsibilities
As an Advanced Nurse Practitioner, you will need to manage your own patients in both a primary care practice setting. Telephone triage may also be required.
You will be required to fully document all aspects of patient care using the EMIS Clinical System and agreed templates and protocols, working in accordance with agreed administrative systems. You will need to send and receive written information on behalf of the practice relating to physical and social welfare of the patients. Work closely with the manager and other members of staff in setting up and improving practice systems for monitoring and measuring performance against clinical governance and quality indicator targets, including those relating to the quality and outcomes framework and enhanced services.
In summary the successful candidate will:
Have experience in General Practice
Have active registration with NMC
Keep your registration and training up to date and source relevant training
Be educated to masters level in advanced nursing
Be an independent prescriber
Meet NMC revalidation requirements
Demonstrate autonomous evidence
Follow agreed policies and procedures
Act upon alerts (eg QOF) during routine visits
Provide all ANP services required
Provide appropriate healthcare for patients presenting with health problems
Provide preventative healthcare and advice
Provide medical support for patients referred by colleagues
Adhere to the practice prescribing formulary, local and national guidelines
Record clear IT based consultation notes to agreed standards
Collect data for audit purposes and participate in clinical audit
Attend and participate in appropriate meetings as deemed necessary by the PM eg. MDT and clinical meetings.
Carry out any other duty deemed necessary for the safe and smooth running of the practice.

Health & safety
The post-holder will implement and lead on a full range of promotion and management of their own and others health and safety and infection control as defined in the practice Health & Safety policy, the practice Health & Safety manual, and the practice Infection Control policy and published procedures. This will include (but will not be limited to):

6. Responsible for the correct and safe management of the specimen process, including collection, labelling, handling, use of correct and clean containers, storage and transport arrangements.
7. Management and maintenance of Personal Protective Equipment (PPE) for the practice including provision, ordering, availability and ongoing correct usage by staff.

12. Making effective use of training to update knowledge and skills and initiate and manage the training of others across the full range of infection control and patient processes.
13. Monitoring practice facilities and equipment in relation to infection control, ensuring that provision of hand cleansing facilities, wipes etc are sufficient to ensure a good clinical working environment. Lack of facilities to be escalated as appropriate.

15. Using appropriate infection control procedures, maintaining work areas in a tidy, clean and sterile, and safe way, free from hazards.
16. Initiation of remedial / corrective action where needed or escalation to responsible management.
