Have you got what it takes tohelp rehabilitate people with longer term Mental Health conditions back intothe community? If so, we want you to apply for the role of Occupational Therapist Associate Practitioner in the newly formed Community Enhancing RecoveryTeam. You would be joining an OT & peer support worker, your work will bedirected by the MDT & influenced by the OT in CERT.
This new service is anintegrated function of the Enhanced pathway & was established to assistpeople who were in longer term hospital back into the community. The team havenow expanded to include people who are being considered for lockedrehabilitation, the aim here is to actively increase their support in thecommunity & to avoid admission. Working with the care coordinators, theright candidate will provide additional support for the person.
The Enhanced pathway offers carefor a service user population from a diverse group, some of whom have longerterm & more complex needs often these people can get stuck in their journeyto recovery.

Main duties of the job
Your role in the team would beto facilitate the care plans developed by the MDT, you would be working closelywith the Occupational therapist & the peer support worker alreadyestablished in the team. You will also work directly with the care coordinatorsin the enhanced pathway.
The most important skillsnecessary are that you are caring & compassionate & have the patience &ability to listen to the person goals & aspirations. You have to want tomake a difference to someone's life who has complex emotional needs & towork with them in a trauma informed way. The role will require you to create links withlocal community groups & to work as a collaborative part of the MDT aroundthe person.
You will need to follow careplans created by the care coordinator & OT. You will also be required toinput into these at reviews points. You may also be required to in reach tolocal inpatient units.

Job responsibilities
Undertake delegated duties under the supervision of registered health care professionals.Consulting as necessary and working collaboratively within the health care teamto deliver therapeutic interventions, psychological and physical care to adefined group of service users.
Deliver evidence based planned care,which is safe and effective, in a manner which is caring, compassionate andenables service users to reach an optimum level of functioning, independence, rehabilitationand recovery.
Provide care within well establishedpolicies, procedures, protocols, guidelines, and established clinical careplans, whilst acting within the required sphereof clinical competences for the roleat all times.
Followinginitialassessment by aregistered health care professional, exerciseinitiative and judgement in, care planning, goal setting, delivery,implementation and evaluation of care.
Workingcollaboratively and in partnership with others, develop knowledge to promoteholistic care.
Demonstratesound knowledge of the careand treatment programmes of individual service users to enable the post holder to contribute and informthe decision making of registered health care professionals.
Undertakeenhanced training and preparation to meet the specific clinical needs of theservice and to equip the post holder for an enhanced leadership role.
Contribute to enabling service users toreach their potential, including the utilisation of public health approacheseg, providing advice and support or signposting to the appropriate service.
Undertake and maintain the requiredstatutory, mandatory and core training as required by the specific area ofservice.
May be required to work in challengingenvironments which could include using managing aggression and violencetechniques, lone working and conflict resolution.

1. Must be willing/able to undertake all appropriate Trust mandatory training requirements, and subsequent refresher training relevant to the area.
2. Must be willing/able to undergo job development and training and maintain skills.
3. Demonstrate excellent knowledge of service user safety, eg, Prevention & management of violence & aggression, moving and handling, etc.

6. A-C in Maths/English GCSE or equivalent.
7. Level 5 Apprenticeship/Foundation degree in Health & Social Care or equivalent relevant to the post.
8. Or, level 3 Apprenticeship/Diploma in Health & Social Care with the ability to demonstrate competencies at level 5 and will willingness to work towards achievement of level 5 qualification.
9. Care certificate or required to achieve within 12 weeks of commencement (where relevant to the post).
