We are looking for a passionate Senior Adult Nurse Manager to join a well-established and progressive provider of complex care packages within the community, specialising in adult complex care. This community role will be based in the Basingstoke area where the company has secured further packages during an exciting growth phase.
The successful Senior Adult Nurse Manager will be responsible for providing leadership and support to a team of nurse case managers in the region as well as providing face-to-face support to a reduced caseload of adult clients and their families, whilst creating personalised care plans for each person. You will work closely with a core team of carers, training and supporting them with their quality, clinical governance and supporting them with daily activities.
Criteria for a Senior Adult Nurse Manager:
1. RGN with an active NMC PIN
2. Supportive management style that can bring out the best in others
3. Previous experience working in complex care in the community
4. Up to date experience of tracheostomy care and ventilators within the last 2 years
5. Advanced communication skills verbally and written to liaise effectively with patients, families and the MDT
6. Suitable for Band 6/ Band 7 Nurses with evidence of effective leadership and management of HCAs and nurses
7. Full UK Driving License desirable but not essential
Key responsibilities for a Senior Adult Nurse Manager:
1. Design and deliver a tailored care plan specific to an individual client's needs
2. Effectively manage the MDT through strong communication and teamwork
3. To provide ongoing monitoring, support and assessment of client's physical and physiological needs within your caseload
4. To deliver nursing interventions including the management of medicine delivery
5. Build and maintain sophisticated relationships with your client's family
6. Working closely with community teams, CHC nurses and CCG commissioners to establish and maintain bespoke packages of care
Benefit package for a Senior Adult Nurse Manager:
1. Up to £55,000 per annum DOE
2. 1 day working from home per week / 4 days in the community
3. Monday to Friday (9-5) hours
4. 33 days holiday
