Responsibilities:
* Respond promptly and effectively to referrals, assessing the needs and risks of individuals and families.
* Carry out comprehensive assessments and develop appropriate plans in collaboration with clients and other professionals.
* Provide ongoing support and intervention to individuals and families, ensuring their well-being and safety.
* Collaborate with internal and external stakeholders to coordinate services and resources for clients.
* Monitor and review cases regularly, making timely decisions regarding appropriate actions and interventions.
* Maintain accurate and up-to-date case records and documentation in compliance with organisational standards and legal requirements.
Requirements:
* 2 years post qualifying experience
* Being SWE registered
