Advanced Specialist Mental Health Pharmacist – Community Rehabilitation Enhanced Support Team (CREST)
An exciting opportunity has arisen for an enthusiastic and self-motivated pharmacist with experience in mental health to further develop their skills and knowledge in the specialist area. You will support our Community Rehabilitation Enhanced Support Team (CREST). CREST aims to provide a holistic, multidisciplinary rehabilitation function for patients with complex Psychosis and severely impaired social and everyday functioning. This will be in accordance with the NICE guideline of Rehabilitation in Complex Psychosis, GIRFT Mental Health Rehabilitation, and the NHS Long Term Plan. We aim to deliver rooted in the community where the patient resides, specialist psychological treatment for complex psychosis, tailored occupational therapy interventions, social engagement work and family work.
As a psychiatric clinical pharmacist the post holder will deliver a high standard of clinical pharmaceutical care and be a key member of the multidisciplinary team providing expertise in medicines related issues. The post holder will support the medicines optimisation agenda within own area of specialty.
There are opportunities to practice as an Independent Non-Medical Prescriber so we will support you in your professional development, including gaining the Independent Prescriber qualification.
Main duties of the job
1. To provide high quality, specialist clinical pharmacy service to CREST ensuring safe, clinically effective and cost efficient use of drugs
2. To work as part of the MDT to develop understanding of the needs of patients with complex psychosis in the CREST service
3. To ensure best practice in medicines optimisation
4. To deliver on clinical advice and audit
5. To provide clinical pharmacy advice on the management of patients with mental illness in order to maximise benefit and minimise risk in relation to medicines
6. To deliver education and training on mental health pharmaceutical issues to a variety of different health and social care professionals both within the CREST service model, the wider integrated care system, primary care and external stakeholders.

The role is part-time (up to days per week) and based at London House, Bracknell. There are opportunities for working from home for this job role.

20. Experience in mental health hospital pharmacy or community mental health team/service
21. Self motivated, confident and ability to navigate conflicting work priorities
22. Have a flexible approach to work in order to meet the developing needs of the pharmacy service
23. Ability to work autonomously providing patient facing and non-patient facing clinical advice

Education/Qualifications/Training
Essential criteria
24. Good Pharmacy Honours degree (BPharm or MPharm)
25. Clinical or psychiatric postgraduate certificate
26. Clinical or psychiatric postgraduate diploma (Psychiatric preferred)
27. Registration with the General Pharmaceutical Council (GPhC)
Desirable criteria
28. Independent Prescriber (Non-medical Prescriber) or willingness to undertake the course to achieve NMP/IP qualification
29. Member of the Royal Pharmaceutical Society
30. Membership of the College of Mental Health Pharmacy
Continuous Professional Development
Essential criteria
31. Evidence of continuing professional development
32. Compliance with the GPhC CPD on-line recording system
Previous Experience
Essential criteria
33. Substantial post registration experience as a practising pharmacist
34. Substantial experience of mental health community and / or hospital pharmacy
35. Excellent working knowledge of mental health therapeutics and prescribing issues in mental health
36. Experience of multidisciplinary mental health team working and making clinically appropriate interventions
37. Experience of line management
38. Experience and competence to provide patient information & education to patients and carers
39. Experience in audit and project facilitation
40. Experience in carrying out medicines information queries, providing both verbal and written specialist advice Ability to demonstrate an understanding of the key stakeholders the post is likely to be supporting and how they will be expected to support them
