Job summary
We have a vacancy for a Practice Nurse to join our friendlyteam. We are looking for somebody to work 35 hours per week over 4 days.
We are a progressive SystmOne practice with a list size ofapproximately 12000 patients. We have an excellent nursing team consisting of 3nurses and 3 HCAs. Working alongside them are 4 GP partners, 3 ECPs & 2 PracticePharmacists and a highly supportive Patient Service team. We are a trainingpractice for GP registrars and medical students and highly value every memberof our practice team.
We are looking for someone who can deliver a large range ofpractice nursing services.
Salary will be dependent upon experience.
Main duties of the job
To be responsible for the delivery of practice nursingservices, working as part of the practice multidisciplinary team, deliveringcare within their scope of practice to the entitled patient population. Thepractice nurse will be responsible for a number of clinical areas such ashealth promotion, chronic disease management, health prevention, well women andwell man clinics, as well as supporting the management team in the reviewing ofclinical policy and procedure.

Job responsibilities
a.Develop,implement and embed health promotion and well-being programmes
b.Implementand evaluate individual treatment plans for chronic disease patients
c.Identify,manage and support patients as risk of developing long-term conditions,preventing adverse effects on the patients health
d.Provideroutine nursing care to patients as required in accordance with clinical basedevidence, NICE and the NSF
e.Providewound care according to dressings and wound care /Doppler etc.) to patients
f.Providetravel medicine services
g.Requestpathology services as necessary
h.Processpathology results as required
i.Providechronic disease clinics, delivering patient care as necessary, referringpatients to secondary / specialist care as required
j.Maintainaccurate clinical records in conjunction with extant legislation
k.Ensureread codes are used effectively
l.Maintainchronic disease registers
m.Develop,implement and embed well woman clinics.
n.Develop,implement and embed well man clinics.
o.Chaperonepatients where necessary
p.AssistGPs with coil clinis when required.
q.Prioritisehealth issues and intervene appropriately.
r.Supportthe team in dealing with clinical emergencies.
s.Recognise,assess and refer patients presenting with mental health needs.
t.Implementvaccination programmes for adults and children.
u.Supportpatients in the use of their prescribed medicines or over the counter medicines(within own scope of practice)
v.Liaisewith external services / agencies to ensure the patient is supportedappropriately (vulnerable patients etc.)
w.Delegateclinical responsibilities appropriately (ensuring safe practice and the task iswithin the scope of practice of the individual)
x.Supportthe clinical team with all safeguarding matters, in accordance with local andnational policies.
y.Understandpractice and local policies for substance abuse and addictive behaviour,referring patients appropriately.
