Main duties of the job

We are actively seeking a motivated and versatile nurse to join our clinical team, dedicated to maintaining the delivery of top-notch nursing care to our patients. You will work as part of the practices clinical team and be responsible for delivering a high standard of nursing care to patients. You will have excellent clinical skills, ideally with experience across most aspects of nursing in general practice or a willingness to expand your knowledge and skills, including:
- Chronic disease management (LTCs)
- Cervical screening and womens health
- Childhood immunisations
- Travel immunisations
- Wound care
- Phlebotomy
- Ear irrigation
- Family planning
- New patient health checks

Job description
Job responsibilities
Clinical responsibilities:
Implement health promotion and wellbeing programmes
Develop and implement individual care plans for chronic disease patients
Manage and support patients at risk of developing long-term conditions, providing advice to promote the patients health
Provide wound care
Run specialist clinics such as ear syringing, contraception etc.
Collect specimens for pathology
Provide travel advice and vaccinations
Request pathology as necessary
Process and interpret limited test results as required ( cervical cytology results)
Provide LTC clinics and refer patients to secondary care as required
Maintain accurate contemporaneous clinical records in line with current regulations
Ensure data is coded appropriately
Implement an effective call/recall system
Implement an effective immunisation programme in line with the national schedule
Chaperone patients as necessary
Support the clinical team in dealing with clinical emergencies
Contribute to practice targets (QOF, local targets, etc.), complying with guidance as necessary
Liaise with external services and agencies to ensure vulnerable patients are supported (safeguarding)
Supplementary responsibilities:
Provide nursing expertise and guidance as necessary
Act as the infection prevention and control lead for the practice, ensuring compliance with all relevant legislation and regulations, undertaking periodic audits as necessary
Monitor and ensure the safe storage, stock rotation and disposal of emergency drugs and vaccines
Support and participate in shared learning within the organisation
Support and participate in the practices significant event reviews and processes

3. Met the requirements for and completed revalidation with the NMC
4. Evidence of continuing professional development (CPD) commensurate with positions held

5. Experience of working in general practice as a practice nurse
6. Experience of working autonomously
7. Comprehensive understanding of infection prevention and control measures
Desirable
8. Experience across the range of primary care nursing duties, including long term conditions
9. Experience of prescribing and undertaking medication reviews
10. Experience of working in a supervisory or mentoring role
Qualifications
Essential
11. Degree in nursing
12. Registered nurse on the nursing and midwifery council register

13. Qualified independent nurse prescriber
14. Postgraduate diplomas or degrees (in clinical areas such as chronic disease management)
15. Registered for other relevant roles (cervical screening, spirometry)
