Job description
As an Outreach Recovery Worker, you will play a vital role in engaging with individuals in the community who are experiencing mental health or substance misuse issues. You will provide support, encouragement, and practical assistance to help clients achieve their recovery goals and improve their quality of life.
Responsibilities include, but are not limited to:
* Conducting outreach activities to connect with individuals who are in need of support, including visiting clients in their homes or other community settings.
* Assessing clients' needs and developing individualised recovery plans in collaboration with them. Provide practical and emotional support to help clients work towards their recovery goals.
* Acting as an advocate for clients, helping them navigate and access appropriate services, resources, and support networks.
* Maintaining accurate records of client interactions, progress, and outcomes. Regularly review and update recovery plans to reflect clients' evolving needs.
* Responding to crisis situations effectively, providing immediate support and coordinating with other services as necessary to ensure the safety and well-being of clients.
* Working closely with other professionals, including mental health services, substance misuse services, and social care teams, to ensure a coordinated approach to client support.
* Providing information and education to clients and their families about recovery strategies, coping mechanisms, and available resources.
* Contributing to the development and implementation of community-based initiatives and activities that promote recovery and well-being.
Requirements:
* Proven experience in a similar role or within a related field, such as mental health, substance misuse, or social care.
* Strong interpersonal and communication skills, with the ability to build rapport with clients and work collaboratively with a range of professionals.
* Understanding of recovery principles, mental health issues, and substance misuse. Knowledge of local resources and support networks is an advantage.
* Empathetic, resilient, and proactive, with a commitment to supporting individuals through their recovery journey.
* Enhanced DBS dated within the last 12 months. On the update service? You could be fast tracked through our recruitment process today.
* Willingness to complete an enhanced DBS if you do not currently hold one.
