Job Responsibilities
1. Holding a client case load, carrying out Care Act (2014) assessments and care planning to meet assessed eligible needs.
2. Carrying out assessments in hospital or reassessment following hospital discharge to support the Integrated Community Service service purpose.
3. Carrying out Mental Capacity Assessments and Best Interest decisions, Multi-agency meetings, working with partner organisations and colleagues, the person and families to provide effective outcomes that meet the assessed needs of residents of Shropshire.
4. Completing CHC checklists and participating in Decision Support tool meetings.
