Position
An opportunity has arisen for a 14 months Fixed Term General Practice Clinical Pharmacist at North HSCP. The post is based in GP practices across Ayrshire. Occasionally however, you may be required to work in other areas within NHS Ayrshire & Arran.
What you will do:
You will support the delivery of safe, clinically effective and cost effective prescribing by:
1. Providing GP practice based clinical audit and patient facing medication reviews on specific topics promoting high quality prescribing with a focus on improving cost-effective prescribing and supporting the Prescription for Excellence agenda.
2. Providing expert advice on prescribing and medicines use in a broad range of therapeutic areas and clinical settings, to GPs, nurses, other healthcare professionals and non clinical professionals.
3. Providing advice for patients and/or carers regarding medicines-related issues or treatment of minor ailments, signposting to other healthcare professionals where appropriate
4. Developing, implementing and evaluating a range of processes to ensure successful implementation of NHS Ayrshire & Arran prescribing strategies and policies.
5. Developing the General Practice Clinical Pharmacists role. Where qualified and competent to do so, this may include:Prescribing and de-prescribing in response to changes in patients condition, as part of chronic disease management clinic or polypharmacy medication reviewMedicines reconciliation following hospital discharge or attendance at outpatient clinic

Knowledge, training and/or experience required to do the job:
6. Masters of Pharmacy Degree or equivalent.
7. Member of the General Pharmaceutical Council of Great Britain (including evidence of continuing professional development).
8. Vocational Training (Level 2) or training experience to an equivalent level.
9. Relevant pharmacy experience.
10. Experience in primary care pharmacy.
11. Ability to work effectively as part of a team, in a pharmacy and multi-disciplinary environment.
12. Required to demonstrate a highly specialised level of clinical pharmacy knowledge, skills, clinical reasoning and judgement.
13. Requires advanced arithmetic skills.
14. Computer literate with knowledge of information technology.
15. Good knowledge and understanding of relevant standards and guidelines.

Hours:
37 per week
In future, in accordance with Agenda for Change terms and conditions, you may be required to work over 7 days and/or an extended working day.
