We are looking to recruit a proactive, energetic and positive Registered Practice Nurse to join our friendly and enthusiastic team.
You should have a flexible and adaptive approach to working and learning, we are looking for someone who is eager to develop themselves within the Practice.
You will work within the general practice team to meet the needs of patients and support the delivery of the services. You will be expected to demonstrate clinical and communication skills to a high standard and a commitment to improving nursing practice through on-going personal and professional development.
Salary is negotiable dependent on experience.
Full time hrs/ Part time hours negotiable.
Main duties of the job
As a Practice Nurse you will be responsible for the delivery of general practice nursing to the whole practice population. The focus of the role is both the delivery of evidence-based practice for patients presenting with long term condition, and the provision of preventative healthcare to the practice population.

Job responsibilities
To provide andmaintain a high standard of nursing care for patients, as well as providingnursing assistance to the doctors and other members of the primary healthcareteam. The duties will include all tasksnormally undertaken by an experienced RGN and any additional roles agreedbetween the nurse and the doctors as appropriate, having regard to currenttraining.
The main duties of the job
Support the practice in achieving quality indicators and targets of the GMS contract and any relevant National Service Frameworks.
Undertake male, female and family planning assessments, give advice and refer patients appropriately
Perform cervical screening and promote breast awareness
Undertake accredited training in specific chronic disease management as/ or if required. diabetes, respiratory, epilepsy, CHD etc. following appropriate training.
Contribute to the maintenance of the practice diseases registers.
Provide advice on lifestyle issues, assist and support patients in changing their unhealthy behaviours and refer appropriately.
