You will be based within TheMount surgery delivering a mixture of Care home visits, Home visits andclinical administration.
Main duties of the job
KEY RESPONSIBILITIES
1. Triage and treat patients, making any necessary referrals to other members of the practice clinical team.
2. Assessing and treating patients presenting with minor injury or acute illnesses in their own place of residence.
3. Ensure clinical practice is safe and effective and remains within boundaries of competence, and to acknowledge limitations.
About us
This long established practice can trace its roots to the turn of the century. The modern practice originates from the mid 20's and the title "The Mount" is taken from the first purpose-built surgery erected in the early 30's.
In March 1986, The Mount Surgery moved to George Street and absorbed the old Pontnewynydd branch.
The new Mount Surgery is housed in Pontypool Medical Centre, off Town Bridge. It opened in June 2002 and is a modern, spacious, fully computerised practice. With six partners and numerous practice and attached staff, we offer a comprehensive range of primary health care.
New Inn was originally a small surgery attached to the local doctor's house moving to its local Greenfield site in 1975, now rebuilt as the purposeful and attractive New Inn Medical Centre which opened in 1993.
With a dedicated, highly skilled team of clinicians and supporting roles, we pride ourselves on being able to offer the very best in patient care and service.

6. Advise patients on maintaining health and refer to allied services when a health promotion need is identified.
7. Communicate at all levels within the multidisciplinary team ensuring an effective service is delivered.
8. Maintain accurate, contemporaneous healthcare records appropriate to the consultation.
9. Ensure evidenced-based care is delivered at the highest standards ensuring delivery of high-quality patient care.
10. Work within local policies and procedures
11. Enhance own performance through Continuous Professional Development, imparting own knowledge and behaviours to meet the needs of the service.
12. Achieve and demonstrate agreed standards of personal and professional development in order to meet the needs of the service.
13. Participate in the audit process, evaluation and implementing plans and practice change in order to meet patient need.
