Norfolk and Waveney ICB are excited to offer this opportunity and look forward to hearing from you if you are a highly motivated, experienced, and enthusiastic Registered Nurse with a specific interest in compassionate end of life care. You will be required to assess and evaluate patient conditions, in accordance with the National framework for NHS Continuing healthcare, fast track pathway.
You will arrange personalised care, which will require close monitoring. You will be working closely with individuals and their families during a difficult time in their lives. You will be working as part of a team that strives to deliver a high standard of care
You will be expected to undertake face to face visits to complete regular reviews and DSTs where appropriate.
Main duties of the job
1. The successful candidate will join the Continuing Healthcare Fast track team, working with autonomy and holding responsibility for managing and prioritising your own workload against the needs of patients and the service requirements.
2. You will be required to receive, triage, and accept referrals from appropriate clinicians for patients with a rapidly deteriorating condition who may be entering the terminal phase.
3. You will manage an individual caseload, supporting patients and their families with high quality, safe and compassionate care, commissioning appropriate packages of care for patients referred for NHS Continuing Healthcare Fast Track in accordance with Norfolk and Waveney ICB`s policies.
4. You will act as a specialist resource for colleagues and other multi-disciplinary team members to advise on Fast Track completion and appropriateness, supporting colleagues with education, clinical work, audit and service development work.
5. You will need to be an effective communicator to share sensitive condition related information successfully with patients, providers and carers in a timely and understandable manner, using different communication methods to clarify understanding.
