Job summary
Southend West Central Primary Care Network has a fantastic opportunity for an enthusiastic and committed nurse practitioner to join our PCN on a full-time basis, for hours per week. There is flexibility to discuss hours at interview.
The five practices within our PCN include Prince Avenue Surgery, The Valkyrie Surgery, Southend Medical Centre, Dr Bekas Medical Centre and Scott Park Surgery.
The main aim of this role is to increase health and wellbeing of patients through improved screening, identification, and disease management. This will entail a mixture of ward rounds, outreach events home visits and some clinics as directed.
Main duties of the job
1. Clinically examine and assess patient needs from a physiological and psychological perspective & plan clinical care.
2. Assess diagnosis, plan, implement & evaluate interventions/treatments for patients with complex needs.
3. Proactively identify, diagnose & manage treatment plans for patients with, or at risk of developing a long-term condition.
4. Diagnose & manage both acute & chronic conditions, integrating both treatment methods into a management plan.
5. Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national & practice protocols & within scope of practice.
6. Implement & participate in vaccination & immunisation programmes for both adults & children.

Qualifications
Essential
14. A relevant nursing degree.
15. Current NMC registration with an active PIN.
16. Fundamentals of Practice Nursing course (General Practice Nurse Foundation Programme) or equivalent experience or qualifications.
17. Competence to undertake asthma, COPD and diabetic clinics and reviews, holding relevant certificates.

18. A non-medical prescribing qualification

19. Evidence of clinical skills and experience in treatment room consultation, phlebotomy, ECG recording, screening, management of long-term conditions, cervical cytology sampling, vaccinations and immunisations, assisting with minor surgery, dressing and wound care, and recording spirometry.
20. Compassionate manner with an empathetic approach and demonstrable kindness in all aspects of your work.
21. Knowledge, and experience of its application, in supporting long-term conditions.
22. Insight and understanding of current issues in nursing and the wider NHS.
23. Strong consultation skills.
24. An excellent communicator and collaborator.
25. Good working knowledge of SystmOne.
26. A full UK driving licence as this role requires travel between our sites and to patients' homes.
