The Stroke Service in NHS Tayside has undergone significant change with centralisation of stroke admissions for the whole of Tayside in a Hyper Acute Stroke Unit, based at Ninewells Hospital in Dundee. This has led to significant improvements in care as measured by the Scottish Stroke Care Audit (SSCA) and made the staffing model more sustainable. Ninewells is planned to be the Thrombectomy Centre for the North of Scotland and the phased introduction of this treatment has begun, working towards a 24/7 service over the next 3 years. The successful applicant will join the dynamic and innovative team developing and delivering this exciting new service with our Interventional Radiology colleagues. The successful applicant will be involved in the evening on call and weekend rota for the stroke service. In addition, they will contribute to the weekday Acute Medical Unit rota. In addition, NHS Tayside with its associated Health and Social Care Partnerships, is redesigning adult models of care with a particular emphasis on rehabilitation pathways. This post offers the opportunity to work across the whole health and care system to develop new ways of working.
Acute Medicine in Ninewells Hospital consists of a 30 bedded AMU, Ambulatory Assessment Area, 20 beds in Short Stay Medicine and 30 beds across Acute Medicine for the Elderly (run by Medicine for the Elderly). The medical model consists of a successful Prof-to-Prof GP advice line and patient-centred outpatient pathways that have reduced hospital admissions. Inpatient care is provided by acute physicians, acute frailty team, speciality in-reach consultants and a large multidisciplinary team. The highly developed ambulatory service deals with over 20% of referrals. The new model has led to a large increase in front-door discharges, 70% within 72 hours, which has helped reduce bed occupancy in medicine to 90-95%. This success has attracted praise and interest from Scottish Government and other health boards.
