We have a part time substantive vacancy in our integrated Heart Failure service. The post is advertised at Band 7 for an experienced Heart Failure Specialist Nurse or we can offer a developmental Band 6 role with progression to Band 7.
Our team provides specialist care for heart failure patients during acute admissions and in the community. We are passionate about the care we provide to our patients and carers and also in supporting each other.
Main Duties of the Job
As a key member of the Royal Devon University Healthcare NHS Foundation Trust, you will be joining an exceptional team working together to help our patients stay healthy when they need it most.
Job Responsibilities
Come and join our friendly dynamic Heart Failure Clinical Nurse Specialist team based in Exeter and surrounding areas. The role will be predominantly based in the Community with opportunities for inpatient work as well. The post holder will manage a caseload via clinics, home visits, and phone appointments. They will support the patient through their journey, providing self-management guidance, symptom management, medication treatments, psychological support, and palliative care. Our Heart Failure Clinical Nurse Specialist service also delivers a rapid access diagnostic clinic and virtual ward.

Working Pattern: 22.5 hours a week over 3 days.
