About the role:
The Connected Care team is involved in the assessment and support of Connected
Carers. The team is responsible for undertaking viability assessments, Connected
Carer assessments and the support and supervision of Connected Carers upon
approval. The team works closely with the Special Guardianship team.
The team is supportive. The Principal Social Worker will work alongside Senior Social
Workers, Social Workers, Newly Qualified Social Workers, Support Workers and team
coordinators.

About you:
• Be motivated and determined to achieve successful outcomes for children and
young people and their connected carers.
• Have the ability to create supportive and meaningful relationships with
connected carers in fluctuating circumstances, including where there is hostility.
• Possess significant experience of working in a fostering and connected care
background including undertaking connected care assessments and supervision
of connected carers.
• Be organised and have the ability to work to set timescales.
• Have sound assessment and analytical report writing skills.
• The ability to work closely with partner agencies and social work colleagues to
meet shared goals.
What you will do:
• Carry out agreed tasks to assist the Team Manager, such as supervision and
oversight of viability and connected care assessments.
• Management of the viability referral process and support of assessment
allocation.
• Attendance at family network meetings, legal planning and PLO tracker
meetings.
• Undertake assessments and supervision of connected carers with greater
complexity need.
• Take the lead in professional development and reflective practice sessions
within the team; covering areas of interest and need.
• Show a commitment to providing a high standard of social work practice by
meeting our statutory duties and following policy and procedure.
