We are looking for a Practice Nurse to join our large supportive nursingteam at Park Road Group Practice. Experiencedin chronic disease management would be desirable but not essential.

Main duties of the job
The practice nurse, working as part of a multidisciplinary team will be responsible for the implementation of processes for the effective management of patients with long-term conditions using evidence-based practice. You will ensure nursing services are delivered effectively to our practice population. The Practice Nurse will be responsible for a number of clinical areas such as infection prevention and control, chronic disease management, health promotion, wound management, as well as supporting the practice management team in the reviewing and delivery of clinical policies and procedures.

Job responsibilities
Duties which may be included in the role but notexhaustive:
Support the Infection Prevention Control leadand to work with our IPC Policy supporting the implementation of audits,training, support and evidence of compliance to the Clinical Manager.
Assess the needs of patients ensuring the provisionof healthcare is appropriate, incorporating evidence-based practice.
Develop, implement and embed health promotion andwellbeing programmes.
Implement and evaluate individual treatment plansfor chronic disease patients that promote health and wellbeing.
Identify, manage and support patients at risk ofdeveloping long-term conditions, preventing adverse effects to patientshealth.
Identify and manage acute and chronic conditions.
Manage wound care, including leg ulcer management,compression and doppler.
Prescribe medication for patients in accordancewith PGDs and own scope of practice.
Review patient medications to enhance compliance,within own scope of practice.
Coordinate the provision of travel medicineservices.
Request pathology services and process pathologyresults as required.
Oversee the provision of chronic disease,delivering patient care as necessary, referring patients tosecondary/specialist care as required.
Ensure coding of medical records are usedeffectively.
Ensure the appropriate maintenance and use ofchronic disease registers.
Develop, implement, embed and participate inwell-being of patients.
Prioritise health issues and interveneappropriately.
Support the team in dealing with clinicalemergencies.
Recognise, assess and refer patients presentingwith mental health needs.
Implement vaccination programmes for adults andchildren.
Liaise with external services/agencies to ensurethe patient is supported appropriately.
Delegate clinical responsibilities appropriately,ensuring safe practice and the task is within the scope of practice of theindividual.
Support the clinical team with all safeguardingmatters in accordance with local and national policies.
Monitor and ensure safe storage, rotation anddisposal of vaccines, medicines and consumables.
Support junior members if the nursing team byproviding support and guidance where necessary.
Support and participate in shared learning.
Participate in local initiatives to enhance servicedelivery and patient care.
Participate in the review of significant andnear-miss events applying structured approach.
