Rate Of Pay: £12 Per Hour
Shift Patterns: 40 hours a week permanent contract.

Main function of the job:
To oversee the referrals and admissions into the service, supporting with administration of clinical aspects of the service user pathway through admission. Take a lead on the coordination of patients' discharge to ensure a safe and seamless transition from hospital to home/alternative placement. This role involves effective communication with all disciplines involved in the care of the patient both with the hospital and in the wider community.
Main duties of the role:
* Provide accurate and efficient typing support from written and recorded material, utilising Microsoft Office to ensure the layout and appearance meet corporate standards, maintaining confidentiality at all times.
* Support Clinical Team in the delivery of administration tasks, including diary management, preparation and coordination for meetings.
* Design service user-friendly documentation as required.
* Coordinate, prepare, and record documentation for meetings, including drafting, typing, copying, collation, and circulation of papers.
* Set up and maintain an efficient filing system in support of the service, reviewing to meet changing demands in consultation with the registered Manager and administrative support team.
* Assist with the collation, storage, and recording of archive documentation.
* Set up and maintain efficient and effective monitoring systems to include all quality audits; Care Quality Commission reports, safeguarding, complaint, incident/accident reports, and provider visit reports.
* Arrange meetings and travel arrangements as required.
* Maintain client and business confidentiality at all times.
* Overcome barriers to discharge, such as homelessness.
* Liaise with external agencies, such as the Police, Safeguarding, housing, and social care, where required.
* Liaise with local teams to identify appropriate people for individual tasks such as representation at MHRTs.
* Manage the discharge checklist to ensure all tasks are undertaken in a timely manner.
* Support the MDT to identify the nominated Care Coordinator and/or Social Worker for each patient on admission.
* Ensure effective communication with Care Coordinators regarding discharge dates, times, and destination.
* Liaise with local teams to confirm appropriate transportation has been booked and confirmed.
* Ensure that invitations are sent to relevant agencies for participation in meetings.
* Use good working relationships with ward staff to facilitate speaking to the relevant person on the Unit for completing care assessments.
* Focus on patient experience throughout their admission to Magna House.
* Facilitate safe and effective discharge for patients by using tools and systems available.
* Work within a quality assurance framework to help deliver high-quality care.
Personal Specification:
* Excellent communication skills, both written and verbal.
* Excellent Microsoft Word, Excel, and Outlook skills.
* Advanced internet use skills.
* Previous experience required: at least 3 years.
* Ability to work autonomously and within the MDT.
* Ability to effectively work to limited deadlines.
* Ability to source information, make contacts, and maintain business relationships with external stakeholders. Knowledge and experience of working with vulnerable adults is an advantage.
