We are recruiting for a Registered Nurse to join our team at Bradford Hospice on our REACT service.
Hours: Full time – 37.5 hours per week
Salary: AFC BAND 5 £29,970- £36,483, per annum, plus enhancements

This new service aims to prevent unnecessary admission to hospital for patients with unmet palliative care needs presenting in the Emergency Department. You will work as part of a multidisciplinary community team providing high quality, integrated, well-coordinated nursing support to patients discharged from the Emergency Department whose needs can be met by rapid response short-term intervention.
You will use evidence-based practice to assess, plan, implement and evaluate patient care supporting a team of healthcare assistants to ensure appropriate clinical care is delivered. The ability to communicate compassionately and effectively is essential as is a clearly demonstrable person-centred approach.
What we’re looking for:
* Registered Nurse on NMC Register
* Experience in NHS or other relevant care setting, ideally in palliative care
* Be person-centred and compassionate when caring for the person and those important to them
* Ability to develop and evaluate care management plans alongside other healthcare professionals
* Proven understanding of communication barriers and enablers
* Individuals with a full driving license and willingness to travel.
