Abbey Medical Group is looking to recruit a Practice nurse to help support our team on a part-time permanent basis.
Abbey Medical Group is a diverse training practice with 12,000 patients, over two sites based in Ravenshead and Blidworth, Nottinghamshire.
The successful candidates will contribute significantly to the nursing team rota for both the Extended Access services and our core opening hours.
Salary will be discussed at the final interview stage and will be based on previous experience.

Main duties of the job
This forward-thinking GP surgery is offering an experienced Practice Nurse the chance to join their dedicated clinicians, who provide nothing but the best service for all patients. You will be part of the nursing team running a range of different clinics. In addition to the standard Practice Nurse qualifications (Including Travel/Child immunisations and Cervical Screening), you must also be NMC Registered and ideally have a minimum of two years experience within a GP setting.
The post holder will use their own judgement resourcefulness andcommon sense and show flexibility with regard to rotas and working patterns

Job responsibilities
Thefollowing are the core responsibilities of the practice nurse. There may be, onoccasion, a requirement to carry out other tasks. This will be dependent uponfactors such as workload and staffing levels:
Primary responsibilities
a. Todevelop, implement and embed health promotion and well-being programmes
b. Toimplement and evaluate individual treatment plans for chronic disease patients
c. Toidentify, manage and support patients at risk of developing long termconditions, preventing adverse effects to patients health
d. Toprovide routine nursing care to patients as required in accordance withclinical based evidence, NICE and the NSF
e. Toprovide wound care (ulcer/Doppler etc.) to patients
f. Toprovide travel medicine services
g. Torequest pathology services as necessary
h. Toprocess pathology results as required
i. Toprovide chronic disease clinics, delivering patient care as necessary, referringpatients to secondary/specialist care as required
j. Tomaintain accurate clinical records in conjunction with extant legislation
k. Toensure read codes are used effectively
l. Tomaintain chronic disease registers
m. Todevelop, implement and embed well woman clinics
n. Todevelop, implement and embed well man clinics
o. Tochaperone patients where necessary
p. Toprioritise health issues and intervene appropriately
q. Tosupport the team in dealing with clinical emergencies
r. Torecognise, assess and refer patients presenting with mental health needs
s. Toimplement vaccination programmes for adults and children
t. Toliaise with external services/agencies to ensure the patient is supportedappropriately (vulnerable patients etc.)
u. Todelegate clinical responsibilities appropriately (ensuring safe practice andthe task is within the scope of practice of the individual)
v. Tosupport the clinical team with all safeguarding matters in accordance withlocal and national policies
w. Tounderstand practice and local policies for substance abuse and addictivebehaviour, referring patients appropriately
x. To deliver opportunistic health promotion whereappropriate
Secondary responsibilities
Inaddition to the primary responsibilities, the practice nurse may be requestedto:
a. Support juniormembers of the nursing team, providing guidance when necessary
b. Participate in localinitiatives to enhance service delivery and patient care
c. Support andparticipate in shared learning within the practice
d. Continually reviewclinical practices, responding to national policies and initiatives whereappropriate
e. Participate in thereview of significant and near-miss events applying a structured approach, root cause analysis (RCA)

Qualifications
Essential
1. Registered nurse.
2. Experience of working in a primary care environment.
3. Experience of chronic disease management.
4. Wound care/removal of sutures and staples.
5. ECGs.
6. Venepuncture.
7. New patient medicals.
8. Chaperone procedure.
9. Requesting pathology tests and processing the results, advising patients accordingly.
10. Travel medicine.
11. Diabetes.
12. Hypertension.
13. Asthma.
14. Spirometry.
15. CHD.
16. Immunisations (routine, childhood and travel).
17. Womens health (cervical cytology, contraception, etc.).
18. Understanding the importance of evidence based practice.
19. Broad knowledge of clinical governance.

21. Ability to work within own scope of practice and understand when to refer to GPs.
22. Understanding of infection prevention and control measures.
