Job summary
Magdalen Medical Practice based in Norwich, is seeking to recruit a care coordinator to join their busy team on a full or part-time basis.
The post-holder will provide extra time, capacity and expertise to our practice team to help us to improve the care provided to our patients.
Main duties of the job
If you are a skilled care co-ordinator, with excellent person-centred planning experience and strong local knowledge for accessing key support, then we would love to hear from you.
The ideal applicant will have the following skills, experience and qualifications:
1. Experience of working within healthcare, the voluntary or community sector, to support vulnerable groups.
2. Familiar with local resources and services, including how to access them.
3. Motivated by helping people, with care and empathy.
4. Able to deliver person-centred support in a non-judgmental way.
5. Experience of completing holistic person-centred care planning assessments.
6. Strong listening and communication skills.

Magdalen Medical Practices history goes back to just after the First World War and the partnership has held on to the best traditions of family medicine while embracing important modern developments.
The surgery is spacious, comfortable and modern. It lies in the north of the city, a quarter of a mile beyond Magdalen Gates. It is a single-storey building with a car-park, easy access via automatic main entrance doors, hearing loop and facilities for the disabled. Our catchment area includes Catton, Sprowston, Spixworth and Heartsease.
