Job summary
The post holder will work within the PCN Team, providing coordination, administrative and support to theclinical teams and other members of the post holder will be primarily responsible for coordinating the PCN response to the Directed EnhancedService requirements across PCN GP practices. Within this there will be a strong focus on leading projectwork and collation of practice data. It will include identifying and understanding patient cohorts, patientengagement and administrative tasks that support the smooth and effective running of the PCN. Theadministrative support to the primary care network, will include activities such as organising meetings,recording minutes and action will need to have excellent telephone communication to support our patients and also externalproviders such as care homes and safeguarding teams. You will also be required to have excellent IT skills,be able to work in a fast-paced environment and work well under pressure. We need you to be enthusiasticand will be expected to work as a team but also using your own initiative. Ideally, we are looking forsomeone who has experience with SystmOne, as well as having a good understanding of healthcare,however full training, and development, will be provided.
Main duties of the job
This role will sit within the broader PCN primary care team as an extension of the wider provision available. This role requires the individual to be able to assess patient needs, helping patients to gain and use knowledge, skills and confidence to become active participants in their care so that they can reach their self-identified health and wellbeing goals.
The role will act as a first point of contact for patients coming into the service where the post holder will assess the patients engagement level coming into the service. For further information and guidance on the main duties for the role please see the attached job description
