The post holder will work as a member of the In-Patient based Care of the Older Person Team at St Margaret of Scotland Hospice and will require specialist knowledge of Care of the Older person. The role will require the In-Patient based Care of the Older Person Nurse to assess and address the needs of patients (with life-limiting illnesses) and their carers through direct/indirect care and multidisciplinary working.
To work closely with members of the multi-professional team within St Margaret of Scotland Hospice and the wider community projects/services to provide specialist care of the older person within the Hospice setting.
To participate in daily Ward Rounds, weekly Grand Ward Rounds and daily Patient handovers within the Specialist Care of the Older Person Centre Mary Aikenhead Ward.
To establish oneself as a resource, facilitator and role model for Specialist Care of the Older Person Care within the Hospice in-patient setting.
To adopt a multi-professional and collaborative team approach to care assessment and delivery to ensure the needs of community-based patients and families are met.
To participate in the Hospice Virtual Clinic and/or deputise for the Care of the Older Person Lead as and when required.
To liaise with the Hospice Social Worker to optimise the psychosocial aspects of the patient and family from a quality-of-life perspective.
To liaise with the Hospice Therapy Team to optimise the functional aspects of the patient and family from a quality-of-life perspective.
To support families/carers in bereavement, referring appropriately to child/young person and adult Bereavement Services, should long-term support be required.
