Job summary
The post is a 10 PA post new post that is split betweeninpatient rehabilitation units across North and South Durham localities. Therehabilitation services serving the North & South Durham localities operateprimarily from the in-patient units at Primrose Lodge (Chester-le-Street and is15 bedded) and Willow Ward, West Park Hospital (Darlington and is 15bedded). There is a provisional plan forPrimrose Lodge is due to relocate to Jubilee House. Jubilee is an 8 bedded ward(4 males, 4 females) based at Shildon, County Durham. The post is created to supporta motivated Multidisciplinary team dedicated to patient care and to promoterecovery.
The main acute in-patient beds for North Durham are atLanchester Road Hospital, Durham and the main acute in-patient beds for SouthDurham are at West Park Hospital, Darlington. There are two wards admittingpatients with acute mental health problems at the Lanchester Road Hospital andcurrently three wards admitting patients with acute mental health problems atWest Park Hospital.
In the North Durham locality, the community mental healthteams are based at Lanchester Road hospital Durham, Derwent Clinic Shotley Bridge,Chester-le-Street Health Centre and Merrick House Easington. In the SouthDurham locality, the main community bases are West Park hospital, the GoodallCentre, Bishop Auckland and Green Lane, Spennymoor.
Main duties of the job
To complete new patient assessment for patientsadmitted to Primrose Lodge and willow wards, including taking a thoroughhistory, mental state examination and complete physical examination in linewith the trust policy. This also included making an initial assessment of risks,and forming an initial management plan in collaboration with the admittingnurse.
To ensure continuous monitoring of physical healthof patients admitted to primrose lodge and willow ward and where necessary toarrange further investigation or referral as required.
To attend the 24 hour Morning Report Out meetingwhen key decisions are made about patientcare and the management plan, risk and leave arrangements are reviewed.
To complete medication review and ensure ongoing monitoring.
Prescribingand medication management
To attend and fully contribute to the formulationmeetings, MDT reviews and discharge planning meetings.
To liaise with community, inpatient, and specialistservices within the Trust as is clinically appropriate.
To adhere to good and timely record keeping,including making specific comment about mental state examinations, riskassessment, management and the plans agreed.
To complete timely transfer of care documents
Mental Health Act assessments (for section 12 approved)
To complete mental health tribunal reports and present evidence totribunal/manger meetings.
To participate in the local on-call rota.
