Supported Assistant Practitioner : £21,422 to £23,327 per annum (Scale F)

An exciting opportunity has arisen for an enthusiastic practitioner to join The Northern Learning Disabilities Team based in North Norfolk. Covering North Norfolk and Broadland our service provides an integrated Health and Social Care approach to assessment and support in the community. About you You will be able to work under pressure, be well organised and able to effectively prioritise. You will need to have a positive attitude and enjoy finding creative and innovative solutions. You will have excellent communication skills, record keeping and keyboard skills. You should have a commitment to excellent customer service and multi- agency working. You will build strong partnerships with service users and their families, be ambitious about what people can achieve and confidently explore ideas for positive outcomes. Who are we? Norfolk County Council and Norfolk Community Health and Care Trust deliver together a single service for adults with a Learning Disability. We are an integrated Health and Social Care team providing specialist Learning Disability support to adults in the community. You would be joining an inspiring and dedicated team of Social Care practitioners who are focussed on supporting adults with a Learning Disability to live independent and fulfilling lives. Depending on your skills and experience, you may be offered the post of Supported Assistant Practitioner (Grade F) and you would be expected to complete our ‘ Assistant Practitioner development programme’ in an agreed timescale. The programme objective is to support you to learn your role, develop your knowledge and skills further. Satisfactory completion of the programme would enable your role to progress to Assistant Practitioner (Grade G)
