We are currently seeking a Care Coordinator to join our client’s team in Swindon. This is a permanent contract opportunity, working within a friendly team.
Job Duties:
1. Report to the Service Manager and be responsible for the day-to-day running of the service provision to clients.
2. Produce detailed work schedules for the Care Workers and provide both written and verbal instructions/guidance to them
3. Complete staff rotas; send rotas to staff ensuring continuity of care. Ensure there are no gaps and staff have enough travel time between calls
4. Assist with recruitment, induction, training, and supervision of Staff.
5. You will be given key clients and it will be your responsibility to keep their information and documentation up to date.
6. Dealing with any compliments and complaints, ensure these are in writing and followed up
7. Enter payroll hours checking against the rota, send completed spreadsheet on the last day of the month to the accountant.
8. Carry out risk assessments under the Health and Safety requirements as directed by the Service Manager.
9. Keep the Service Manager informed of the outcomes and issues identified concerning Health and Safety issues.
10. You will participate in the on-call rota-this can be flexible based on the needs of the company, your needs and the needs of colleagues.
Requirements:
11. Previous experience in a care coordination role or similar position is preferred.
12. Knowledge of the Care Standards Act 2008 and other relevant legislation governing the provision of care services.
13. Excellent communication skills, both written and verbal.
14. Strong attention to detail and organizational skills
15. Ability to work independently and as part of a team.
16. Proficient in using computer software and systems.
17. Full UK driving license.
18. Enhanced DBS Check Required (Can be done upon successful placement)

Pay:
21. £24,000 – £27,000 Per Annum
