Job Title: Social Worker - Hospital Team

Rate: £27.57 per hour
We are looking for dedicated Adult Social Workers to join our Hospital Team in Hammersmith and Fulham, offering a pay rate of £27.57 per hour.
What will your responsibilities be?
In this role, you will conduct comprehensive assessments of patients' social, emotional, and environmental needs. You will develop and implement discharge plans in collaboration with patients, families, and healthcare teams. Additionally, you will manage a caseload of patients, ensuring effective and timely interventions, and monitor and review care plans to adapt to patients' changing needs.

Social Work England registration
Eligible to work in the UK
Hold a full UK licence
Degree level or equivalent in Social Work
