We are seeking a dedicated Practice Nurse to complement our healthcare team. The Practice Nurse will play a crucial role in providing high-quality patient care within our practice. The ideal candidate will have a background in Practice Nursing.
Main duties of the job
Duties:-
1. Conduct patient assessments and provide General Practice Nursing care in accordance with practice policies
2. Administer vaccinations, treatments provide management of chronic diseases and perform cervical cytology screening, spirometry etc
3. Educate patients on health management and preventive care strategies
4. Maintain accurate patient records and ensure confidentiality- Collaborate with interdisciplinary team members to optimise patient outcomes

The permanent role will be either Full or Part-time (depending on the candidate)

Job responsibilities
The post holder is responsible for the delivery of general practice nursing to the whole practice population. Delivery of evidence-based practice for patients presenting with a long-term condition and the provision of preventative health care to the practice population.
As an autonomous practitioner, the nurse is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making in the management of patients. They will work collaboratively within the general practice team to meet the needs of patients and support the delivery of policies and procedures.
Key responsibilities
5. To perform cervical cytology screening
6. To administer childhood immunisations
7. Chronic Disease management Diabetes, Asthma, COPD
8. To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice
9. To develop, implement and embed health promotion and wellbeing programme. Evaluate individual treatment plans for chronic disease patients that promote health and wellbeing
10. To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health and, as required, to diagnose and manage acute and chronic conditions
11. To provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF
12. To request pathology services and process pathology results as required
13. To oversee the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required
14. To implement vaccination programmes for adults
15. To support the clinical team with all safeguarding matters, in accordance with local and national policies
