As a Social Worker within the Children with Disabilities Team, you will work with children and young people who have a variety of complex needs, including physical and developmental disabilities. Your responsibilities will include assessing their needs, creating individualised care plans, and facilitating access to services that enhance their independence, well-being, and overall quality of life. This role focuses on delivering child-centred support while collaborating closely with families and multi-agency professionals to achieve the best possible outcomes. Additionally, you will ensure compliance with statutory obligations under the Children Act 1989 and other relevant legislation, with a focus on safeguarding and promoting the welfare of children with disabilities.

1. Families Act 2014 to understand the needs, risks, and strengths of children with disabilities.
2. Collaborate with families, health professionals, and education providers to gather holistic information
3. Manage a caseload of children with a range of disabilities, ensuring regular review and adjustment of care plans.
4. Identify and respond to any safeguarding concerns, ensuring that children are protected from harm, abuse, or neglect.
5. Advocate on behalf of children and young people to ensure they have a voice in decisions about their care and support
