Responsibilities:
– Conduct assessments to understand patients’ psychosocial needs and preferences before discharge.
– Coordinate and plan comprehensive discharge strategies with healthcare teams.
– Arrange post-discharge services and provide education and emotional support to patients and their families to ensure a smooth transition from hospital to home or another care setting.
What we are looking for in a Social Worker:
4. Social Work England registration
5. Eligible to work in the UK
6. Hold a full UK licence
7. Post-qualified experience
8. Degree level or equivalent in Social Work
